A fast, yet effective way to rejuvenate yourself.  This massage is done with the busy business professional in mind and typically lasts 15-45 minutes concentrating on the head, neck, shoulders, back, arms and hands or 60-90 minutes concentrating on the entire body.

I have been contracted with Rockwell Automation to provide my massage services to ALL employees and contractors who work for Rockwell Automation since
December 2012.

• 10 minute Consultation: FREE
• 30 minute Corporate Massage: $45.00 (plus tax)
• 45 minute Corporate Massage: $65.00 (plus tax)
• 60 minute Corporate Massage: $85.00 (plus tax)

In the state of Ohio, Massage Therapists are required to collect sales tax on all services rendered. Sales Tax rates vary by county and are based on where the service was performed.

In order to save money on your massages

1. If you plan on using a Health Savings Account (HSA) or Flexible Savings Account (FSA) credit card to pay for your massage, I will need to have a prescription written from an MD, DO, Chiropractor or Dentist on file for you. Please make sure that the prescription lists any diagnosis codes so I can be sure to add that to your receipt.
2. If you would like to be exempt from paying sales tax and do not have a Health Savings Account (HSA) or and Flexible Savings Account (FSA) to pay for your massages, I will need to have a prescription written from and MD, DO, Chiropractor or Dentist on file for you. The Prescription can state, “Recommended for massage.”

Prescriptions are good for one year from the date that they are signed. Thirty days before the prescription expires, you will receive an email from me stating that your prescription will be expiring.

At the end of the calendar year, I’m willing to print off a statement with all of your massage appointments listed along with the date of service and the amount you paid if you request it to be reimbursed for your massage appointments.

Feel free to print off this one-page referral form for your doctor to sign. Make sure they list any diagnosis codes in the notes section of this form.
