In the state of Ohio, Massage Therapists are required to collect sales tax on all services rendered. Sales Tax rates vary by county and are based on where the service was performed.

In order to save money on your massages

  1. If you plan on using a Health Savings Account (HSA) or Flexible Savings Account (FSA) credit card to pay for your massage, I will need to have a prescription written from an MD, DO, Chiropractor or Dentist on file for you. Please make sure that the prescription lists any diagnosis codes so I can be sure to add that to your receipt.
  2. If you would like to be exempt from paying sales tax and do not have a Health Savings Account (HSA) or and Flexible Savings Account (FSA) to pay for your massages, I will need to have a prescription written from an MD, DO, Chiropractor or Dentist on file for you. The Prescription can state, “Recommended for massage.”

Prescriptions are good for one year from the date that they are signed.

Feel free to print off this one-page referral form for your doctor to sign. Make sure they list any diagnosis codes in the notes section of this form.
